Eight step guide to the Heart and Mind Health Check

Reliable health risk assessment can be affected by optimistic disclosure of activity levels, diet, family history and alcohol consumption and pessimistic disclosure of mood¹

We elicit accurate disclosure of health risk factors by:

  • Establishing trust and rapport 
  • Listening without judgement and acknowledging your feelings
  • Helping you take a balanced view of your mood during the weeks leading up to the day of your check by careful positioning of questions and probing for underlying symptoms

¹ The Channel 4 “Embarrassing Bodies” programme found that mood pessimism amongst 25% of 1 million self-assessments resulted in an incorrect assessment of mental health risk such as anxiety or depression

  • Height / waist ratio (body shape) and weight (Body Mass Index)
  • Blood pressure, pulse and atrial fibrilation
  • Good and bad cholesterol and glucose (involving  small skin prick)

All measurements are taken in compliance with clinical best practise guidelines using regularly calibrated medical equipment

Cardiovascular Health Risk.  Your Cardiovascular Health risk score is calculated by the QRisk2 algorithm. It is NICE recommended for a full formal assessment of your risk of developing cardiovascular disease in the next 10 years.  Click on the logo to find out more

Type 2 Diabetes Risk.  Your Type 2 Diabetes risk score is calculated by an algorithm developed by Diabetes UK in collaboration with the University Hospitals of Leicester NHS Trust. It is NICE recommended for use by GPs and other primary healthcare professionals to predict your risk of developing Type 2 Diabetes within 10 years.  Click on the logo to find out more

Alcohol Related Health Risk.  Your Alcohol Related Health Risk score is calculated using the AUDIT-C algorithm (Alcohol Use Disorders Identification Test). AUDIT-C is sponsored by the World Health Organisation.  Click on the logo to find out more

Anxiety Risk.  Your Anxiety risk score is calculated using the GAD-7 tool for assessment of the four most common anxiety disorders (Generalized Anxiety Disorder, Panic Disorder, Social Phobia and Post Traumatic Stress Disorder)

Depression Risk.  Your Depression risk score is calculated using the PHQ-9 tool for assessment of depressive symptoms not explained by substance use or another medical or psychiatric condition

This is a non-judgemental and motivational conversation supported by fact sheets on subjects such as: Sleep, Keeping Active, Walking, Blood Pressure, Diabetes, Cholesterol, Alcohol and Smoking.

The fact sheets have evolved out of hundreds of plain English, jargon free conversations.

You may consider changes such as stopping smoking or reducing drinking, increasing physical activity and losing weight.

We feed your lifestyle change intentions into the expert system which re-calculates your health risk scores.

We might have a “what if” conversation supported by the expert system, modifying your intentions and testing their impact on your health scores before concluding on a final set of reasonable lifestyle change intentions.

The conversation we’ve had so far naturally leads onto discussing personal coping strategies such as handling the effects of day to day stressors, signposting to self-help information resources and possibly referrals for further help.

Our Work Wellness practitioner will recommend links to additional resources, sign-posting to local contacts and referrals to specialist practitioners if they are likely to help and support you in sustaining positive lifestyle changes.

Referrals to your GP are supported with a personalised letter detailing the nature of the referral for you to hand over at your GP appointment.

By the end of your Health Check we’ll have discussed a lot of information, answered many of your questions and will be feeling positive and motivated. We don’t want you to lose that feeling once you’re back at your workplace.

So you’ll be able to take away a personalised report containing 15 pages of information you’ve provided, the measures we’ve taken, the lifestyle changes we’ve discussed, their impact on your health risk scores, where to find self-help resources and referral routes to get further help. That’s a lot of information to digest, so your report carefully structures it alongside plain English guidance.

Your Work Wellness Heart and Mind Health Check may not have found any significant health risks.  Alternatively, a Work Wellness referral to your GP or another specialist practitioner may mean that you are receiving some form of treatment

In any event, we encourage you to come back for a follow-up check every year. Our Expert System remembers you and re-presents your previous and latest information side by side, thereby freeing up time for a conversation about your progress, any issues and next steps.
